Patients will be allocated a ‘usual GP’ but this does not mean that you can only see that particular doctor.
If for any reason you are unable to do an online submission, then both forms required are in printable format from the Forms section accessible from the home page under Key Information. We can also email the forms to you if you prefer, please contact us at parkparadesurgery@nhs.net to request this.
